Termly SEND Survey to Parent/Carers – January 2020



Who is the survey for?

It’s for parents and carers of children and young people with special educational needs and/or disabilities (SEND), who have had an Education Health and Care Assessment, or an autism assessment with the NHS.
Who’s asking?

This survey is being carried out by the 0-25 SEND Partnership, which includes all agencies working with children and young people with SEND and parent/carer representatives.
Why do you want to know?

We want to know how SEND processes such as the Education, Health and Care needs assessment and autism pathways have worked for you and what difference they have made to your child, so we can help improve things for others who need support.
Who will read my answers?

Members of the 0-25 SEND Partnership may get a copy of your answers, but they will not know who wrote them.
What are you going to do with my answers?

We will use them to help improve the experiences for children and young people with SEND. We will use the data to inform where we are improving and where we have further work to do. The answers will also be used to write reports that may be made public.
Do I have to answer the questions on my own?

No, you can ask someone you trust to help you complete the survey.
Do I have to answer the questions?

No, this survey is voluntary and if you do not want to take part then that is absolutely fine.
