NHS Health Checks Awareness Survey

Introduction

We are seeking your views on NHS Health Checks to help us improve awareness, access, and overall experience of the programme.

NHS Health Checks are a free service for people aged 40–74 who do not already have certain long-term conditions. The check helps identify your risk of developing conditions such as heart disease, stroke, diabetes, high blood pressure, and high cholesterol. It involves simple measurements (like blood pressure, cholesterol, and BMI) along with a short discussion about your lifestyle, such as smoking, diet, and physical activity.

This survey aims to understand:

  • How aware people are of NHS Health Checks
  • Whether people have accessed them and their experiences
  • What would make it easier to attend
  • Where people would prefer to receive a check
  • What information and support would be most helpful afterwards
  • Your feedback will help us shape how NHS Health Checks are delivered locally, making them more accessible, convenient, and useful.

Please respond by 25th June 2026. The survey should only take a few minutes to complete. All responses are anonymous unless you choose to provide your details for follow-up involvement.

Your confidentiality is assured
