Reason for this questionnaire
Cheshire East Council and the
Cheshire East Integrated Care Partnership has a vision to improve the health and well-being of local communities.
With your help, we would like to explore what aspects of our children’s services we might be able to deliver outside of hospitals, as that might make them easier to access, and might make them more child-friendly. These may be referred to in the future as ‘Child Health Hubs’.
The following questions ask about what health services you have used with your baby/child, about whether they met your needs, and also where you would like to access help and support when you need it.
Your views and opinions will help shape our plans to deliver services in a more integrated and sustainable fashion, closer to your home.
Please note we are
NOT proposing to remove children’s services from any of our hospitals, all of the services that need to be in a hospital will remain there and will be available to you should you need them.

This questionnaire should take no more than 10 minutes to complete, please complete it by
Friday 17 September.
